Specifically for our Client, we have developed a unique Care Management Platform that has helped them to improve collaboration within medical teams and deliver patient-centered care. The use of our system has enabled the Client to ensure effective and hassle-free workflow management using the following features:
  • Home page — a comprehensive dashboard that displays scheduled events, tasks, patient interactions, etc., designed to help medical staff plan their work time wisely and make sure important tasks aren't overlooked;
  • Task management — allows the user to create tasks, assign them to medical staff, set priorities, write notes, and more;
  • Patient profile — all patient data in one place: personal and clinical information, care programs, results of medical exams, contacts, documents, etc.;
  • Electronic signature — allows patients to sign documents such as consent and HIPAA authorization forms online;
  • Medical appointment scheduling — the user can view and manage all scheduled appointments, create reminders, write notes, and more;
  • Patient intake — provides a convenient digital format for gathering valuable patient information to make diagnoses and determine care programs;
  • Omnichannel communication — provides multiple communication channels (email, phone, SMS, live chat, etc.) and displays patients' communication preferences to improve and personalize physician-patient interaction;
  • Notifications — the user can create notifications not to miss something important: to create a task, to call a patient back, to confirm a visit, etc.;
  • Patient progress tracking — provides physicians with a full picture of patient progression over time by visualizing the entire patient journey in the form of charts and graphs.
